Service Options Using Resources in Community Environments (SOURCE)

Elderly and Disabled Waiver
For adults ages 65 and older, unless disabled
Must have a chronic condition that has been present for at least 3 months and must meet nursing home level of care.
Must be eligible for SSI coverage not exceeding $7

Comprehensive Support Waiver

Serves individuals’ with intellectual and developmental disabilities who choose to remain within the community. Individual should be diagnosed before the age 22 and the diagnosis should be indefinite with an IQ <70. Individual should have 3 or more functional impairments such as Self-Care, Learning, Mobility, Self-Direction, Capacity for independent living or Requiring 24 hour care.

New Options Waiver (NOW)

Serves individuals’ with intellectual and developmental disabilities. The NOW provides the following services:

Adult PT/OT/ST
Behavioral Supports
Community Living Supports
Community Access
Prevocational Services
Supported Employment
Specialized Medical Supplies
